NORTHERN RESPITE CARE REFERRAL POLICY

POLICY NUMBER : 12.6

POLICY STATEMENTThe Northern Respite Care Service (NRC) will endeavour to refer all enquiries it receives onto other suitable service providers.

 

EXPECTED OUTCOMES:

  •  A clear process will be developed and documented in regards to the onward referral of enquiries that do not meet the funding guidelines of the Northern respite care Service.
  • Persons making care enquiries will be referred onto other appropriate services

 

PROCEDURES

(To implement Policy)

1. When an enquiry is received by the NRC program for a service request that the program is not funded or able to undertake, the NRC Manager will gather as much accurate information from the enquirer as possible, in order to refer the person onto the most appropriate alternative service.

2. Upon obtaining a clear picture of the type of service the enquiry is seeking, the NRC Manager will provide as much accurate information as possible about alternate services. This information shall be sourced from documents which include the ‘HACC Directory of Respite Care Services in the Northern Region’ and ‘Directory of HACC Low Level Services in the Northern region’

3. If the enquirer is requesting specific information on nursing home accommodation, the NRC Manager will forward a copy of the DPS Guide to Aged Care (SA&NT) book which details nursing homes in South Australia.

4. A spread sheet of all enquiries noting date, enquirer details and recommendations made is to be kept by the Manager NRC for reporting purposes
